manner seems most appropriate. However, the following
information will help us in the documentation process:

- your address, pre-Katrina
- your address, post-Katrina
- your current phone
number
- your year of birth
- make-up of your immediate family
- what preparations
did you make prior to the storm?
- where did you ride
out the storm?
- did other family members evacuate?
